New Patient Registration

Our list is open and we are delighted to accept new patients to the practice.  Please ask for a registration form at reception. The completed form along with proof of id and address will be required to register you.  Following your registration you will be required to have a new patient health check to enable your registration to be completed. An appointment for this will be offered with our healthcare assistant.

If you wish to join and are unsure about your address being in our practice area then you can ring us to check, or to check your postcode against the catchment area on-line please follow this link Map Page (primarycare.nhs.uk)

Please note that it does take up to a number of months for the Health Authority to collect and provide us with your medical file from your previous surgery. Therefore when you do attend for your check up please bring any medications, repeat lists or health information you may have with you.

Medical treatment is available from the date of registration. Please contact reception for further information.
